難聴をきたす疾患の中には, 手術や薬物療法により聴力の改善が期待できるものが存在する. しかしながら現在の医療では聴力改善が困難な疾患も多く, 日常生活における音声言語でのコミュニケーションに支障をきたしている. 治療困難な難聴に対する聴覚の補助として, 補聴器や人工内耳医療は発展してきた. 最新のデジタル処理技術により補聴器の適応範囲は広がっており, 難聴の適切な診断, 治療はもちろんのこと, 聴力の的確な評価と適切な補聴器フィッティングも耳鼻咽喉科医にとって重要な役割であ … る. 補聴器によっても聴取能力改善が乏しい場合には, 人工内耳医療が聴力再獲得に貢献している. 当科での術後聴取能検査では, 単語, 日常会話文ともに平均で75%程度の聴取能力が獲得できており, 高度難聴者のコミュニケーション能力改善に優れた効果が認められる.<br />Some diseases that cause hearing loss can improve the hearing level by surgery or drug therapy. However, many other diseases are difficult to improve the hearing level by the current medical technology, and that makes communication in everyday life difficult for these patients. To improve difficult-to-treat hearing loss, hearing aids and cochlear implantation surgery have been developed. The latest advances in digital processing technology have led to a wider adoption of hearing aids by patients with severe hearing loss who experienced no benefit from conventional hearing aids. Not only an appropriate diagnosis and treatment of the hearing loss, but also appropriate hearing aid fitting and precise evaluation of the resultant hearing gain are also important for the otolaryngologist. If the improvement in hearing with the hearing aids is poor, cochlear implantation surgery may contribute to the re-acquisition of hearing ability. Postoperative hearing ability tests are frequently performed in our department, and the average percentage of correct answers was about 75 % (n = 23) for both words and daily sentences. Excellent benefits in improving communication skills for patients with severe hearing loss have been observed following cochlear implantation surgery. 続きを見る
