概要: Transmission of infectious diseases by the administration of blood, its components or derivatives has been known since the beginning of transfusion. Cases of such transmission have increased with the striking increase of the use of blood produscts in the past ten years. Specially, the use of albumin and fresh frozen plasma (FFp) increased fifty times during that period. The risk of transmissible disease is extraordinarily high for blood collected from paid denors by commercial blood banks. Recently, more than ninety percent of Japan's supply of blood products comes from abroad. We tested for the anti-AIDS virus antibody in blood products sold by ELISA, IFA and western blot, and detected 62% positive in anti-HBs immuno globu-lin (HBIG) on and after 1982. Most commercial globulin preparations are still made by ethanol fraction. Since ethanol concentration greater than 20% inactivate the AIDS virus in vitro, there is no need for alarm forrecipients of current globulin preparations. Globulin administration has never been identified as a risk factor for AIDS despite of the extensive use of globulin. However, we hope to eliminate anti-AIDS virus antibody positive blood from blood products and to make manufacturers of blood products aware of the need for careful donor selection. 続きを見る

概要: In order to investigate the incidence of posttransfusion hepatitis nonAnonB (PTHNANB), 521 patients undergoing cardiovas cular surgery at our department were retrospectively studied. Pediatric patients were divided into 4 groups (ASD group, VSD group, other simple anomaly group, and complex anomaly group), and ASD group received the least blood products (7.75±3.75units/pt.), whereas complex anomaly group received the most (24.6±20.5 units/pt.). The incidence of PTHNANB ranged from 1.1% to 9.3% among the groups, and 3.8% for all the pediatric cases. Adult patients were also divided into 4 groups (ASD group, valvular disease group, ischemic heart disease group, and aortic disease group), and the total volume of blood products administered during the hospital stay ranged from 4.03±7.97 units/pt. (ASD group) to 68.8±77.1 units/pt. (aortic disease group). The incidence of PTHNANB was 16.7% for all adult patients, and the incidence rate went up as the blood products administration per patient increased. Since January 1989, we have started to employ non-blood prime cardiopulmonary bypass and an autotransfusion system whenever it is feasible to save homologous blood transfusion. After then, the incidence of PTHNANB among adult patients decreased from 18.0% to 9.5% in 1989. 続きを見る

概要: Post transfusion Hepatitis (PTH) is one of serious side effects and some times lead to fulminant hepatic failure in case transfused blood contain very low level (under the sensitivity of usual screening method) of hepatitis B virus (HBV). Redcross blood center and blood transfusion devision of our hospital have been employed reverse passive hemmaglutination method (RPHA) for HBsAg screening. Authors employed EIA for sensitive HBsAg test system and compared with RPHA method. Of 2,255 sera from blood bonors, 21 sera showed HBsAg by EIA. Of those 21 HBsAg sera, 4 were negative for HBsAg when tested by RPHA. So about 20% of HBsAg positive sera containing very low level HBV and RPHA could not detect them. For the prevention of post transfusion type B hepatitis, authors changed the HBsAg screening method to EIA. Relationship with so called non-A non-B hepatitis and anti-HCV was investigted. Anti-HCV was detected frequently in patients clinically diagnosed as non-A non-B hepatitis, and correlation of anti-HCV positivity and blood transfusion indicates the importance of the anti-HCV screening in blood transfusion. 続きを見る
