Since 1879, nitroglycerin is used as a remedy for angina pectoris. There is no doubt as to the effectiveness of nitroglycerin in the relief from the pain. The mechanism of anti-anginal effects of nitroglycerin is exclusively thought to be dependent on its vasodilatatory effects. The emerging consensus strongly favours the involvement of cyclic GMP as the mediator of vascular smooth muscle relaxation elicited by nitroglycerin. It is believed that accumulated cyclic GMP activates cyclic GMP-dependent protein kinase (G-kinase), G-kinase phosphorylates target proteins and then relaxation is occurred. Two main pathways are proposed as the relaxation mechanisms, 1) lowering intracellular free Ca^<2+> concentration and 2) reducing C^<2+> sensitivity to contractile proteins. Mechanism 1) will be mainly discussed. The vasodilatory effects of nitroglycerin shows a tissue (vein vs. artery) and a region (large vs. small artery) selectivity. The precise mechanism of the selectivity is not known so far. The region selectivity of nitroglycerin on the canine coronary artery may be based upon the difference of guanylate cyclase activity. 続きを見る
