概要: 要介護高齢者は,嚥下機能の低下などから誤飲などの危険性も高く, 訪問診療で義歯を製作する際には,患者および歯科医療スタッフへの負担も大きい。その結果, 義歯を新製作せずに現状の義歯を調整することで急場をしのぐことも多いが, 根本的な解決に至 らない場合も多い。本研究では, 診療の効率と安全性を高める目的で, Silicon-Model-Systemを応用し, 訪問診療において, 要介護高齢者の総義歯を製作した。その結果, 患者, 術者双方への負担が少ない安全な術式で, 質のよい義歯を製作することができた。すなわち本法では, 歯科技工士との良好な連携のもと, 複製義歯形態の個人トレーを用いることにより, 印象・咬合採得が安全かつ容易に行えた。また, 口腔内での概形印象採得を省略できたこと, および印象材に粘膜調整材を用いたことで, 患者側に与える印象時の苦痛を最小限とし, 誤飲を防止することができた。歯科医師にとって, 現代社会のニーズに応え, たとえ制約が多い訪問診療という環境下にあっても, 質の高い義歯の製作に取り組み, 患者のQOL向上に努めることは極めて重要な任務である。本法を応用することで, 身体機能が低下している要介護高齢者にも安全で質の高い義歯を製作できることが示された。<br />Many elderly people who require nursing care have dysphagia, and sometimes suffer from aspiration pneumonia. Therefore, both dentists and patients often feel stress during denture fabricating treatments at home-visits. To avoid such aspiration, we sometimes choose not to fabricate new dentures, but just to adjust the dentures the patients are using at the time. In this study, We examined a new method for fabricating complete dentures for senile patients who require nursing care in home-visits.We used the Silicon-Model-System to increase clinical effiency and improve safety in the treatments. In applying this method, we succeeded in fabricating new dentures safely with minimal stress for both the dentists and patients. Through this method. the procedures of both impression and bite taking became safe and easy by using the special individual trays like copy dentures, with the efficient cooperation of the technicians. Furthermore, we were able to minimize the danger of aspiration in impression taking, by omitting the primary impressions inside the patients' mouths and selecting a tissue conditioner with little fluidity as the final impression material. In response to social needs, it is essential that dentists provide top-class denture treatment in order to improve the quality of life of their patients, even within the considerable limitations of home-visits. This study proved that the Silicon-Model-System is extremely useful in the denture fabricating procedure for senile patients with restricted physical abilities. 続きを見る

概要: Purpose : Recently, many telemedicine systems have been put into practice, but there are still only few reports in the d ental field. Therefore, our group of specialists from some departments of Niigata Univrersity planed a trial use of a teledentistry system. The purpose of this study was to clarify the effective form of this teledentistry system by conducting a questionnaire survey on dentists in the Niigata prefecture. Methods : We conducted a preliminary questionnaire survey on all members of Niigata Prefecture Dental Association (a total of 1310 dentists), and analyzed the following major items : the field that was needed in teledentistry system : the utilization of information equipments ; the number of consultations for patients with temporomandibular disorders in their clinic ; the utilization of Internet. Results : From the results of this analysis, we found out that : 1. Many dentists recognized the necessity of the teledentistry system in the field of oral surgery. 2. Many dentists selected e-mail in the teledentistry system. 3. The temporomandibular disorders patients were visiting hospital dental departments in greater numbers as compared to private offices. 4. Ninety percent of dentists were using the Internet. The teledentistry system is scheduled to be established based on these results.<br />1.研究目的 近年、医科領域では動画、生体情報、放射線画像、病理画像を通信することによる遠隔診断支援システムが実用化されているが、歯科領域ではまだ報告が少ない。そこで、我々は、本学各診療科専門医が協力し、主に顎関節症に対する診断や治療法に関して遠隔診断支援システムの構築を計画した。そのために予備的調査を行い、診断支援の必要性と適切な情報交換の方法について検討し、運用可能な遠隔診断支援の形式を明らかにすることを目的とした。 2.方法 予備的アンケートの調査対象者は、新潟県歯科医師会全会員1,310名である。なお、2名以上の歯科医師がいる医療機関については、代表者1名に回答していただいた。調査内容は、回答者に対する質問事項として、性別、年齢、職場等である。診断支援に関する質問事項として、診断支援を希望する分野(顎関節症治療および他の歯科治療全般について)、診断支援に利用を希望する通信方法、診査・診断機器の利用状況、顎関節症患者の来院状況、診断支援の必要性、インターネットの利用状況等についてであった。 3.結果 診断支援については、口腔外科、顎関節症、隣接医学、歯科放射線、口腔病理など口腔外科に関連した分野の希望が多かった。診断支援の際に利用を希望する通信方法については、電子メールの希望者が多かった。診査・診断機器の利用状況については、顎機能検査機器の利開者は少なかった。顎関節症患者は個人開業医よりも病院歯科に集まる傾向が明らかになった。診断支援の必要性については、必要性を認めている回答が多かった。インターネットについては、回答者の90.1%が利用していた。今後、この結果に基づいて、診断支援システムを構築してゆく予定である。 続きを見る

概要: Two cases of obstructive sleep apnea syndrome treated conservatively by a oral appliance are reported. Case 1 was a 60-y ear-old man with elongated soft palate and narrow posterior air way space. On polysomnography, apnea and hypopnea were observed 94 times and 13 times, respectively and ST depression was noted during apnea on ECG. The minimum SpO2 was 68% and SpO2 decreased below 90% 52 times and for 1470 seconds during sleep. Because of myocardial infarction, surgical treatment was abandoned and a oral appliance was set to put the mandible anteriorly by 7mm and posteriorly by 6mm. It resulted in an increase of the posterior air way space. The minimum SpO2 became 86%. The number and time of SpO2 below 90% also decreased to 23 times and 390 seconds, respectively. Respiration disturbance during sleep was markedly improved. No complications associated with the appliance were encountered. Case 2 was a 49-year-old man who had sleep apnea due to excessive elongation of the soft palate and narrowing of the posterior air way space. Apnea and hypopnea were encountered 75 times and 32 times during sleep, respectively. The minimum SpO2 Was 56% and SpO2 decreased below 90% 92 times and for 1985 seconds. All these values were aggravated just before hemodialysis due to thickning of the soft palate. The mandible was set anteiorly and posteriorly by 6mm. Then, there was a 1.9mm increase of the posterior air way space which was accompanied by an improvement of SpO2 and disappearance of nocturnal respiration difficulties. The dental appliance was found to be effective in the treatment of obstructive sleep apnea syndrome.<br />口腔内装置で保存的に治療した睡眠時無呼吸症候群の2例について報告した。第1症例は、60歳男性で軟口蓋が長く咽頭気道の狭窄が認められた。睡眠ポリグラフで無呼吸数および低換気数は、それぞれ94回と13回を観察し、無呼吸時、心電図でSTの低下が記録された。睡眠中の最低SpO2は68%で、SpO2が90%未満となった回数は52回、その時間は1470秒であった。心筋梗塞を合併しているため外科治療は断念し、下顎を前方へ7mm、下方へ6mm移動した位置で歯科補綴装置を装着した。これにより、咽頭気道径が拡大する結果となった。最低SpO2は86%になった。SpO2が90%未満となった回数と時間はそれぞれ23回、390秒となった。睡眠中の呼吸障害は明らかに改善した。装置装着による副作用は認められなかった。第2症例は、49歳男性で軟口蓋が長く咽頭気道の狭窄があるための睡眠時無呼吸であった。睡眠中の無呼吸数および低換気数は、それぞれ75回と32回を観察した。最低SpO2は56%でSpO2が90%未満となった回数は92回、その時間は1985秒であった。これらの値は透析直前には軟口蓋が厚くなるために悪化した。下顎位は前方、下方に6mmの位置で装置を装着した。これにより咽頭気道径が1.9mm増加し、SpO2の改善と夜間の呼吸障害が消失した。口腔内装置は、閉塞型睡眠時無呼吸症候群の治療に有効であると思われた。 続きを見る

概要: To clarify the control mechanism for jaw movements, we developed an autonomous jaw-movement simulator, JSN/1, consisting of upper and lower jaws, temporo-mandibular joint, muscular actuators, periodontal sensors, control unit, and a computer. The muscular actuator consists of a cable-tendon DC servo motor, equipped with a cable-tension sensor, and a rotary encoder capable of detecting cable length, thereby providing JSN/1 with masseter, temporalis, lateral pterygoid, and digastric artificial muscles. The actuators were controlled adaptively under an impedance-control mechanism, using bite force, and tooth contact data, plus the tension and length of the actuator cable. We also developed a physiological adaptive control scheme for open-close movement of JSN/1, using EMG data for related muscles. The previous simulator, however, had a defect: the temporalis actuators were activated under a non-physiological position control during closing in order to obtain high reproducibility of the movement. To eliminate that defect, we first incorporated artificial lateral ligaments into the temporomandibular joint of the simulator, in an attempt to provide the condylar movement with a passive restriction. Additionally, the drive signal for the anterior temporalis actuator was updated to cope with a delay of closing movement due to the reduction of the position-feedback gain. This update was accomplished not only by reducing the position-feedback gain, but also by dividing the signal into two different sinusoidal waveforms, each having a control parameter, thus enabling us to modify the signals for closing and biting phases independently. Accordingly, the drive signals for the masseter actuator and internal-pterygoid actuator, newly incorporated in the JSN/2B, were modified so as to be activated immediately after tooth contact. Their position-feedback gains were also reduced. The second aim of this study was to establish a simulator control scheme for chewing-like jaw movement without food, as a preliminary study for the simulation of actual chewing movement. We assumed that such movement can be obtained merely by modifying the values of several control parameters of the digastric, lateral-pterygoid, and posterior temporalis actuators, determined adaptively during open-close movement, and that the values of the parameters can be optimized using data relating to the amount of lateral shift of the mandible. Experiments using the updated JSN/2B verified that a lifelike, reproducible open-close movement could be obtained in the simulator even without high position-feedback in the actuator control, demonstrating both the necessity of joint ligaments for stability of condylar movement and the validity of our control scheme. In other experiments on chewing-like jaw movement, we introduced two different optimization functions to determine the values of the control parameters, using bite-force sensor data on the working side and tension sensor data for the lateral ligaments. Empirical results using both optimization functions demonstrated that a lifelike, reproducible chewing-like jaw movement could also be obtained by slightly modifying the values of the control parameters, which were first optimized during open-close movements. 続きを見る
