examined, using avidin and anti c-kit antibody for mast cell staining, Ulex europaeus agglutinin-1 lectin, anti α-smooth muscle actin antibody and anti type IV collagen antibody for blood vessels, propidium iodide for nuclear staining, anti E-selectin antibody for E-selectin expression, and anti IgA antibody for immune complex (IC). The relationships among IC deposition, inflammatory cell infiltrate, E-selectin expression in endothelial cells and mast cell degranulation in the disease were analyzed with confocal laser scanning microscopy. IgA is always deposited mainly in capillary loop, but in most of the lesions examined only a small amount of inflammatory cell infiltrate is observed in the blood vessels. E-selectin was expressed in endothelial cells of blood vessels in fifteen of thirty-two patients (46.9%). Most blood vessels with E-selectin-positive endothelial cells were the horizontal plexus, and dense inflammatory cell infiltrate was always observed around the blood vessels. In lesions from seven patients the number of avidin-binding cells around blood vessels was inversely proportional to the density of inflammatory cell infiltrate. In historogically uninvolved site from four of them, the number of avidine-binding cells was significantly decreased compared with normal skin. However, there was no significant difference in the number of c-kit positive cells between involved and uninvolved vessels from all of the four patients. In addition, fluorescence intensity of avidin-binding granules was much decreased in involved vessels compared with uninvolved one. In capillary loop in skin lesions from nine patients, there was no difference in the number of avidin-binding cells between lgA positive and negative vessels. Fluorescene intensity of avidin-binding cells was markedly decreased around the E-selection-expressed vessels. These data suggest that (1) mast cell degranulation occurres during the early stages of vasculitis, even before the inflammatory cell infiltrate ; (2)IgA-containing immune complexes per se is not sufficient for mast cell degranulation; (3) E-selectin-expression, which is localized mainly in horizontal plexus, is associated with must cell degranulation and may induce infiltration of neutrophills.

ial sepsis, myocardial infarction, and heart failure and it decreases myocardial contractility. However, its electrophysiological property is unknown. We here report that TNF-α inhibits cardiac delayed rectifier pottasium current (IK). METHOD：Ventricular myocytes isolated from guinea pig hearts were voltage-clamped by conventional whole-cell mode (34-36℃). Amplitudes of tail and steady-state (2s pulse) currents were measured as IK. Intrapippete and extracellular K concentrations are 150 mM (pCa＝8) and 5.4 mM, respectively. To minimize L-type Ca currents, nisoldipine (2μM) was usually added to extracellular solution. RESULTS：TNF-α suppressed the IK by 98.4±1.9% that had been enhanced to ～2-fold by isoproterenol (ISO；20nM), although it did not affect the basal IK. The action was dose-dependent with IC50 of 11.6±0.69 ng/ml (n＝23). TNF-α antagonized histamine- and forskolin-enhanced IK (n＝4), but failed to inhibit that enhanced by internal application of cyclic AMP (n＝4). Preincubation of myocytes with PTX (5μg/ml for>120 min) abolished the inhibitory action of TNF- α on ISO-enhanced IK(n=4). Preincubation of myocytes with N-oleoylethanolamine (NOE) ceramidase inhibitor, abolished the inhibitory action of TNF-α on ISO-enhanced IK (n=4), and C_2-ceramic inhibited ISO-enhanced IK as well as TNF-α did. CONCLUSION: TNF-α inhibited IK in a dose-dependent manner by modulating cAMP concentration, and PYX-sensitive G protein and sphingomyelin pathway were concerned with the inhibitory action on ISO-enhanced IK. Under pathological conditions, TNF-αmay prolong the action potential duration, thereby leads to intracellular Ca overload.

esophagitis-related symptoms manifested in 84.7% of those patients. Heart burn and regurgitation frequently developed and reduced quality of life postoperatively. Proximal gastrectomy, as well as total gastrectomy, and jejunal interposition were associated with high incidences of reflux-related symptoms. Overall, quality of life of the patients improved according to the lapse of time after gastrectomy although incidences of reflux esophagitis-related symptoms were unchanged by the time lapse.
